Registration of Past Birth under act 24 

1. National Identity Cards (Pre-Processing)

6. Valuation Certificates
   
7. Proof of Residency Certificates
   
8. Leaving of Residence Certificates
   
9. Certificates to Obtain Water Facilities
   
10. Certificates to Obtain Electricity Facilities
    
    

Registration for new NIC 

1. Completed application form 01 of department of Individual registration should certify by Grama Niladari.
2. Applicant should be 16 years old.
3. Should paste Rs.13.00 value stamps.
4. Should attach original and certified true copy of birth certificate by Grama Niladari.
5. If applicant is old more than 50 should present a letter certify by GN giving reasons to delay with an oath statement.
6. Need 02 more extra photos other than attached 03 photos in the application.
7. If applicant is more than 50 and no birth certificate should present predicted age certificate or certificate registered as a post dated birth.
8. If applicant is less than 50 and no birth certificate should present vacant result sheet with children’s birth certificates.
9. If needed all documents are completed application form 13 of department of Individual registration should noted and should present

to AO/AG for certification

New NIC (Behalf of lost NIC)

1. Police report (should not exceed 01 year).
2. Should paste Rs.15.00 value stamps to the application form.
3. Should attach original and copy of birth certificate.
4. If applicant is more than 50 and no birth certificate should present vacant result sheet with children’s birth certificates.
5. If applicant is less than 50 and no birth certificate should present predicted age certificate or certificate registered as a post dated birth.
6. Need lost ID number and written proof to used ID. (eg: passport, driving license or bank accounts  certified photo copies).
7. Need 02 more extra photos other than attached 03 photos in the application.
8. Need service letter received within 03 months to include post holding.
9. If using name after marriage need marriage certificate.
10. If needed all documents are completed application form 7 and 13 of department of Individual registration should noted and should

resent to AO/AG and AAG for certification.

Registration FOR amended ID 

1. Completed application form 08 of department of Individual registration should certify by Grama Niladari.
2. Need 02 more extra photos other than attached 03 photos in the application.
3. Should paste Rs.15.00 value stamps to the application form.
4. Old ID
5. Need birth certificate if old ID is not clear.
6. If applicant is more than 50 and no birth certificate should present vacant result sheet with children’s birth certificates.
7. If applicant is less than 50 and no birth certificate should present predicted age certificate or certificate registered as a post dated birth.
8. If using name after marriage need marriage certificate.
9. If needed all documents are completed application form 8 and 13 of department of Individual registration should noted and should

present to AO/AG and AAG for certification.

Receiving a public fund (Asthma, Wheezing, Heart diseases, Fit etc) 

1. Requesting letter
2. Model form signed by applicant (SS/M/A 01).
3. GN report (SS/M/A 02).
4. Medical certificate if suffering from any disease.
5. Approval of Social Services Officer.
6. Approval of AG/AAG.
7. Noted in the list.
8. Approval of Social Services Director.
9. If approved including to paying list if there are funds.
10. If no funds available receiving from department of social services.
11. Issuing written card if AG has signed.
